Annual Review Findings

- Completeness: 94% of required indicators reported (target: 98%). Gaps concentrated in community-based service delivery indicators.
- Timeliness: 87% of reports submitted by the quarterly deadline. Late submissions correlated with staff turnover at implementing partner organizations.
- Accuracy: 3.2% error rate in beneficiary counts (improvement from 4.7% in FY2025). Most common errors: double-counting across service delivery points.
Required Actions
All implementing agencies with data quality scores below 90% must submit corrective action plans by August 15, 2026. Plans should address root causes and include specific training commitments for data entry staff.
